By signing and checking the box, you acknowledge that you have read, understood,
and agreed to all the terms and conditions stated herein.
• Appointments are subject to availability and official confirmation by the clinic.
• For cash payments, bookings will only be confirmed once payment is received before 3:00 PM a day prior to the scheduled appointment.
• Failure to confirm payment after 3:00 PM a day before will result in automatic cancellation.
• Changes or rescheduling can only be made until 2:59 PM a day prior to the appointment.
• Any clinic-initiated changes in schedule are not subject to cancellation fees or penalties.
• Cancellations made after 3:00 PM a day before the appointment will be charged a cancellation fee equivalent to one (1) session.
• Cancellations must be sent through the clinic’s official channels (call, text, or email).
• Emergencies may be considered upon review and approval of clinic management.
• The clinic reserves the right to reschedule sessions due to therapist unavailability or unforeseen events without penalty to the client.
• Clients who fail to appear for their appointment without notice will be considered no-shows.
• No-shows or late arrivals of 30 minutes or more will be charged a full session fee.
• Clients are encouraged to arrive at least 15 minutes before their scheduled appointment.
• Late arrivals will only receive the remaining session time and cannot extend beyond the reserved slot.
• The clinic enforces a zero-tolerance policy for abuse, rudeness, or disrespect toward staff or other clients.
• We may refuse or discontinue treatment if a client displays inappropriate behavior.
• Damages caused by a client may result in billing for repairs and service termination.
• Clients must disclose all relevant medical conditions before treatment.
• The clinic is not liable for adverse effects resulting from undisclosed medical conditions.
• Treatment may be refused if a client shows signs of illness for safety reasons.
• All client information is confidential under the Data Privacy Act of 2012 (R.A. 10173).
• Information is used only for clinical, legal, and administrative purposes related to patient care.
• Clients understand that therapy results may vary and physical therapy involves inherent risks.
• The clinic and staff are not liable for incidental effects unless due to proven negligence.
